Father and sonIf you or someone close to you has ALS, we strongly encourage you to register as a Member with the ALS Society of Quebec.

Among other things, you will be eligible for our equipment loan program, you will have the right to take part in activities reserved for members, and receive literature on the disease. 

To register to the ALS Society, you must download, print, and fill out the form below and return it to us by fax, mail or e-mail.
